Bridges Counseling Center is committed to ensuring the security of your information. To prevent unauthorized access, maintain data accuracy, and ensure the appropriate use of information, we have put in place appropriate physical, electronic, and managerial procedures to safeguard and secure the information we collect online. When submitting sensitive data like credit card information, we use 128-bit secure server technology to protect the information. The secure server software (SSL) encrypts the information you input before you send it to us. All of the data is protected against unauthorized access, as indicated by the secure icon on your browser.
Bridges Counseling Center collects IP addresses for system administration and internal tracking and analysis. When you visit our site, our servers log the IP addresses only. However, through the services provided by SmarterStats, we do receive information on the companies and industries visiting our site based on the IP addresses that are logged.
Bridges Counseling Center values your opinions and comments; we may, therefore, conduct online surveys. Participation in these surveys would be optional. The information would be aggregated and used to make improvements to Bridges Counseling Center and to develop useful content, features and promotions for Web site visitors. Consent to Treat
I understand that in the therapeutic process the therapist provides a safe, confidential, non-judgmental environment to explore and learn about oneself. The therapist and I will collaborate to create positive changes that incorporate my values and beliefs. The therapist’s role involves listening, encouraging, and supporting me, the client.
I understand that all information shared with the clinicians is confidential and no information will be released without my consent. In all other circumstances, consent to release information is given through written authorization. Verbal consent for limited release of information may be necessary in special circumstances. I further understand that there are specific and limited exceptions to this confidentiality which include the following:
- When there is risk of imminent danger to myself or to another person, the clinician is ethically bound to take necessary steps to prevent such danger.
- When there is suspicion that a child or elder is being sexually or physically abused or is at risk of such abuse, the clinician is legally required to take steps to protect the child, and to inform the proper authorities.
- When a valid court order is issued for medical records, the clinician and the agency are bound by law to comply with such requests.
I understand that a range of mental health professionals, some of whom are in training, provides services. All professionals-in-training are supervised by licensed staff.
I understand that my therapist may consult with other therapist regarding my treatment with other colleagues. This is will be done on a regular basis to ensure that I am receiving the highest quality of treatment my counselor can provide.
If I have any questions regarding this consent form or about the services offered, I may discuss them with my therapist. I understand that I may discontinue treatment at any time.
